Sign up for the First Annual MTILP, Inc./Successful Work Options Golf Tournament at Foxboro Golf Club to benefit adults with disabilities in Madison. We would like to raise $3500 for much needed equipment and a new activities room. Please join us for a day of fun and prizes!! Sign-Up must be completed by August 29, 2003.

WHEN: SEPTEMBER 19th 2003, 11:00 free beer, 12:00 pm tee times start.

WHERE: FOXBORO GOLF CULB, 1020 County Road MM. Oregon, WI

WHAT: Two Person Scramble (Both players hit from best ball)

PRICE: $65 per golfer for 18 holes w/cart. Free beer (until its gone) and lunch at
the turn

TOP PRIZE: $200 cash - 1st, $100 cash - 2nd, Plus many great raffles and prizes
There will also be hole prizes on 10 out of the 18 holes and chances to win cash and prizes out on the course.
